Sample Reports

Review sample Independent Medical Examination reports to see how my findings are organized and presented. Each report is prepared in accordance with the AMA Guides to the Evaluation of Permanent Impairment, Sixth Edition, and includes a thorough records review, a detailed history, a focused physical examination, and a clear impairment rating methodology. Reports are written to be objective, well supported, and easy for attorneys, adjusters, and other decision-makers to follow. Three samples, including the full reports, are available below. All patient and case identifiers have been removed or altered to protect confidentiality, and the samples are provided for illustration only.
