Schedule an Appointment
Please follow the below instructions:
Choose the office location: Oklahoma City or Tulsa
Choose the type of examination that matches the body region to be examined.
Pick a date and time, and answer the booking questions. Please do not enter any personal identifying information or medical details.
Each examination is scheduled for 2 hours. If there are multiple body regions to be examined, choose “Multiple Regions” and add expect 15 minutes more for each additional region.
After your appointment is booked we will send you the examinee’s intake packet by email or mail, depending on the contact information you give us. The packet is lengthy and it is very important that it be completed before arrival, so that the full time can be spent on the interview and physical examination. If filling it out by hand it should be completed in ink. If the examinee emailed the packet back prior to the exam they should arrive 15 minutes prior to the appointment time. If the packet is being hand carried in the examinee should arrive 30 minutes prior to the scheduled exam time.
